If you’re feeling a little overwhelmed by all the decisions to be made in relation to your care, we can help you get started. Our support coordinator's help you to find quality support services and will ensure they are reliable and suitable for your needs.

How Support Coordinator's help you

A Zest Support Coordinator knows disability services better than most. This allows them to provide specific recommendations that align to your plan and your individual goals.

Best of all, they work for you! They will search for reliable disability service providers on your behalf to make sure you get the most from your NDIS Plan. If something isn’t quite right, they will make adjustments until you are happy.

A professional Zest Support Coordinator will also help you to manage your disability support services yourself, once you are ready, so you can gain more independence in the future.

Why choose Zest Care's Support Coordination service?

At Zest, we pride ourselves on delivering the best approach for you. We have over 20 years of experience in providing care services to Australians.

You will be assigned a designated support coordinator who continues working for you throughout your entire plan period. They’ll listen to you, get to know your story and put your wishes into action. It’s a partnership that keeps you in control.

Our support coordinators can empower you to organise your supports on your own. They work with you to develop the knowledge, skills and the confidence you need to set and achieve your goals in the future.

What is Support Coordination?

If you’re living with disability, your plan from the National Disability Insurance Scheme (NDIS) gives you a set allocation of funding for a range of different support services.

The support funding in your plan is very specific and relates to helping you to achieve your personal goals. This might include a range of services such as in-home support, social and community access or therapy sessions. It will also state if you are eligible for support coordination.

A support coordinator will simply help to connect you with all the individual services outlined in your plan.
