What to have ready

Have a NDIS number and plan showing the supports and services you are eligible for.
Have a copy of your NDIS plan so that you can find the information we need to set you up.
Have ‘Plan Management’ included in your NDIS plan under Capacity Building Support – Choice and Control or Improved Life Choices.

If you don’t already have it, you can request it from your Local Area Coordinator (LAC) or NDIA representative before you register.

Endorse ‘Independence Solutions’ (trading as Zest Care) as your plan manager and quote our Provider Number: 4050127674.

For participants with new plans created after 30 October 2023 that have access to my NDIS (PACE) portal, please contact the NDIA, your Local Area Coordinator (LAC), NDIS contact, or early childhood partner to endorse Independence Solutions as your plan manager.
